Register for Dip-In 2006

Because of funding cutbacks, we will not be able to send questionnaires for the 2006 Dip-In to your volunteers.  However, we would appreciate it if you would let us know if you will be participating

I will have my volunteers participate in this year's Secchi Dip-In.  Approximately   volunteers may participate.

I will not participate this year

Assuring Quality Data

It is important that the Dip-In data is of sufficient quality that it can be used in scientific studies and by agencies.  Quality data depends on many things, but training certainly contributes to the production of quality data.  In order to have some idea of how much training is given volunteers in contributing programs, would you please answer the following?

Is your data entered into STORET, the Federal Environmental Database?   Yes   No   I don't know

See  http://www.epa.gov/storet/ for more information on STORET

Does your program have a Federally approved Quality Assurance Project Plan (QAPP)?   Yes   No   I don't know

See http://www.epa.gov/owow/monitoring/volunteer/qappcovr.htm for an explanation of a Volunteer QAPP

Does your program have written instructions for the volunteers for each test they perform?Yes   No   I don't know

Does your program conduct training sessions for each volunteer?  Yes   No   I don't know

Please describe nature of the training that each volunteer receives.

Does your program conduct re-training or field quality assurance examinations?   Yes   No   I don't know

Please describe how your program assures quality data

Please tell us more about you and your monitoring program (Please fill in if you have not done so before)

I am a coordinator of an organized Volunteer Monitoring Program
(We define a volunteer program as involving two or more persons that routinely monitor a waterbody on a volunteer basis).

If you coordinate a monitoring program, please answer the next two questions

Number of volunteers in your program

At what level does your program monitor?
                 Multiple State/Province
                Statewide/Province-wide
                Regional (within state or province)
County or multiple waterbodies in local region
Single Waterbody (such as a Lake Association)

I am a member, but not the coordinator, of an existing Volunteer Monitoring Program.  I would like to receive information about the Secchi Dip-In directly.
The name of the volunteer program  of which I am member:

I am participating in the Dip-In as a member of an established professional monitoring program (Agency, County, or City monitoring program) or a professional program that monitors routinely as a part of a grant or project.

I am not a member of an organized Volunteer Monitoring Program.  However, I will submit data to the Secchi Dip-In..

I will be involving a class or group in the Dip-In

Elementary (K-6)
Middle School (6-9)
High School (9-12)
College
Other organization (Boy Scouts, 4-H, etc.)

I do not monitor but am interested in being kept informed about the Dip-In.

My relationship to monitoring is not described above:

          Please describe:

Please feel free to elaborate about your program, its geographic extent, its members, or anything else that you think defines what you do:

Sometimes people enroll as a local volunteer monitoring organization, but they send the data to a state or regional volunteer monitoring program.  We are happy to send reports to you as a  local monitoring program or individual, but we need to know more about who is ultimately receiving this data.  If you are sending the information from your local monitoring program to another monitoring program, please name them and explain the relationship between your groups.

Other Information We Could Collect (Please fill in if you have not done so before)
